2. PENALTY FOR LATE PAYMENT Failure to pay the practising fee for ANY YEAR before
31ST
DECEMBER OF THE PRECEDING YEAR will attract a minimum penalty of
double the Annual Fee, apart from the likelihood of the defaulter being prosecuted.
To facilitate the issuance of the practising licence before the 1st
January, doctors are
advised to pay their fees as early in the year as possible, notwithstanding the 31st
December deadline.
3 Please note that doctors can only renew practising licence ANNUALLY ONLY.
4 Doctors are to submit evidence of 20 units of Continuous Professional Development
/Continuous Medical Education from an accredited CPU provider (s).
5 House officers & resident doctors are exempted from no 4; however they are to show
current evidence of internship/residency from their Chief Medical Directors.

REQUIREMENTS FOR CHANGE OF NAME
1 Formal Application for Change of Name
2 A recent passport photograph
3 Original copy of sworn Affidavit for Change of Name
4 A copy of Marriage Certificate
5 Original copy of Publication in a National Daily.
6 For males, the request for change of name should be accompanied with an effected
change of name on their MBBS certificate.
7 Pay the necessary fees.

Note: Original Copy of degree Certificate (both local & foreign trained doctor) must
be sighted before collection of Full registration Certificate.
REQUIREMENTS FOR REGISTRATION OF ADDITIONAL QUALIFICATION
1 A Copy of Additional Qualification Certificate being registered
2 A Copy of Full Registration Certificate
3 One Recent Passport Photograph
4 Pay the necessary fee.
5 Fully completed application form.
Note: Original Copy of Specialist Certificate (both local & foreign trained doctor)
must be sighted before collection of Full Registration Certificate.
REQUIREMENTS FOR TEMPORARY REGISTRATION OF FOREIGN DOCTORS
1. Letter of Good Standing from Medical Registration Authority in Country of Origin;
2. Copy of Basic Medical or Dental Degree Certificate (MBBS or MBChB);
3. Name and Address of Institution where Basic Medical or Dental Degree Certificate (MBBS or
MBChB) was obtained including website and e–mail address;
4. Copy (ies) of Additional Qualification Certificate (if applicable);
5. Name and Address of Institution(s) where Additional Qualification Certificate was obtained
including website and e–mail address (if applicable);
6. Completion of Application (Temporary Registration and Practising Licence) forms to include
three recent passport photographs and Temporary Registration form should be endorsed
with official stamp from the doctor’s country of origin embassy and stamp from the employer;
7. Letter of appointment from the employer;
8. Evidence of having been previously registered by home country;
9. Expatriate quota (from the Nigeria Immigration office) ;
10. Resident/work permit (from the Nigeria Immigration office);
11. Certified English translation of any document in other language;
12. Certified bank draft of all the appropriate payment fees required;
13. Photocopy of the page that indicate date of entry in the doctor’s international passport
endorsed by the immigration office.
PLEASE NOTE:
• All foreign doctors without a specialist degree would have to sit and pass the
council’s assessment examination before he/she could be registered by the council.
• Doctors with specialist degree(s) would be found registrable by the Council
only after such degrees have been verified to be confirmed authentic by the

awarding institutions and on the list of recognized additional qualifications by the
Council.
